We are delighted that your young person will be joining us for a pamper party. In order for your child to safely attend the party, we need to ask you a few  questions

All persons under the age of 16 are required to have a parent or guardian fill out this form.

We want to provide a safe foundation for your child to be able to enjoy the day. We have an information form below where you can tell us about all of the important information for your child (this includes any medical information, medication, allergies, preferences and adjustments) All information is treated in the strictest of confidence.
